The first step to take to eliminate credit card debt is to stop using your credit cards. Pay your bills and then just use the cash that is left over for your expenses. If you don't have the cash, you don't buy anything.
The second step you should take to eliminate credit card debt is to pay more than the minimum each month on your cards. Choose the card with the lowest balance and pay as much as you can afford to on it each month until it is paid off, while still paying the minimums on the other bills you have. Once you finish with one debt, take the next lowest and throw the money you were paying on the first one into that debt in order to pay it off. Continue in this method until you have paid everything off.
Once you have managed to eliminate credit card debt, you will probably want to cut up all but one or two of your cards with the longest history or the lowest interest rates. You don't necessarily want to cancel the cards since this might negatively effect your credit rating, though if you have a lot you might gradually cancel some of the newer cards over a period of time. The remaining cards you will want to use just enough to keep them active, paying off the balance each month so you don't have to pay any interest and so you don't get in over your head again.
